Adventure Island 2 continues the adventures of Master Higgins, but this time with a new storyline. The game introduces a scenario where Master Higgins must once again embark on a rescue mission, but rather than saving just one person, he is on a quest to save his girlfriend Tina's sister, who has been kidnapped by the evil witch doctor's younger brother. This plot adds a fresh twist and motivation for players as they guide Master Higgins through his perilous journey.

One of the most notable enhancements in Adventure Island 2 is the introduction of dinosaur companions. These creatures, which can be found in eggs scattered across the levels, add a new dimension to the gameplay. Each dinosaur comes with its unique abilities, such as breathing fire, flying, or swimming, allowing players to tackle the game's challenges in varied ways. This mechanic significantly diversified the gameplay, offering more strategies and approaches to the levels.

The game retains the essential platforming mechanics that made the original Adventure Island a success. Players navigate through a series of islands, each filled with treacherous terrain, enemies, and obstacles. The energy meter system returns, requiring players to continually collect fruit to keep Master Higgins' energy levels up, underlining the game's signature blend of platforming action and survival.
